Section 4 - Valuation Forms
Step 1: Enter your business name and the name of the product you’re completing the form for.
Step 2: Break out your material cost (imported), material cost (Hawaii), direct labor cost (Hawaii), manufacturing cost (Hawaii), and creativity value.
Step 3: To get your Hawaii value, add lines B, C, D, E & F.
Step 4: To get your product cost (line H), add line A + line G.
Step 5: Complete the calculation (H-A)/H=I for line I to show your Hawaii Value Added percentage.
Step 6: Line J is your retail selling price.
Step 7: Use the box to describe your manufacturing process for each product category. This section is important for the committee to understand how you manufacture your products in Maui County. You may add additional pages and photos/videos.
